To save time at your first appointment, you can request our new patient forms in advance — Patient Registration, Medical History, Financial Policy, and Notice of Privacy Practices. Just give us a call at (816) 858-2707 and we'll send them over, or arrive a few minutes early to fill them out at the office.
Bring your insurance card, photo ID, and a list of any current medications. If you have x-rays or records from a previous dentist, those are helpful but not required.
